What are the chances that a crystal oscillator would be defective?

if the "crystal" is an orange ceramic resonator i'd say 50/50.
i have replaced them in tvs and remote controls. only the freq was differant.

While it is possible, I highly doubt it. The metal "canned" crystals seldom fail.
No trees were harmed in the creation of this message. But billions of electrons, photons, and electromagnetic waves were terribly inconvenienced!

The oscillator includes a transistor & capacitors which are more likley to fail. The crystal can break from mechanical shock or excessive drive.
