I think the smallest geometrics is a 100 pin VQFP and 32 pin TSSOP. The last the most difficult due to the distance between the rows. It's a good help to be nearsighted , at least when you are as old as I am.

If this is a real machine, I would think they use a small camera to look at and score the dices. The same way they score the pins in a bowling machine. Doesn't take too much software to recognize the result, with proper filtering.

I would also take a look on your detector and filter or screen it from ambient light with 50/60Hz frequency. This optical 'noise' may well ruin your pulse detection, by adding pulses or overloading the detector.

What output voltage is your chip for? What is the output voltage from your micro? Values of capacitors used as decoupling and source?

Have you tried to supply it from the circuits power supply, if so, how does it behave then?

To save power you should pullup all unused inputs via a 2k2-4k7 resistor.
Use the same value for the pullup rsistor on the output from the LM319.

The main problem now is to select the inverter to use.
